Program Manager – Palliative Care
Under the general direction of the CSR Ops team, this position is responsible for overseeing the development, coordination, and administration of all aspects of Palliative Care unit & Pulmonary Rehabilitation Centres (PRC) components of a CSR program.
Palliative care aims to support patients with any serious illness, at any stage of diagnosis. This type of care is focused on providing relief from the symptoms and stress of the illness. The goal is to improve the quality of life for both the patient and the family. Pulmonary Rehabilitation Centres (PRC) to provide free of cost pulmonary rehabilitation services for cases with chronic respiratory disease and long COVID-19.
Pulmonary Rehabilitation is a non-pharmacological treatment that is widely accepted and is used to improve the physical and psychological conditions of people with chronic respiratory diseases.
Duties will include planning, organizing, staffing, and leading program activities.
ESSENTIAL DUTIES AND RESPONSIBILITIES: May include the following & other duties as assigned.
Monitoring programmatic deliverables as per following:
a) To engage with key officials and leadership within the hospital to have buy in for the project
b) To engage with key Dr/ Physicians within the hospital to have project deliverable agreed
c) To identify the right set of people for the project and have consensus on hiring of project staff with the hospital team
d) To facilitate training of project staff beside formal onboarding
e) To have defined working modalities and project indicators for the project staff
f) To provide oversight to the project staff by holding routine meetings
g) To have routine meetings with key officials within the hospital to ensure project outcome is known
h) To provide monthly reporting on projects
i) To have future visibility on the project and progress update
Staff & Lead Program:
Create a volunteer application and contract for participation
Design a volunteer training program
Recruit and train a volunteer workforce
Coordinate the delivery of services among different program activities to increase effectiveness and efficiency
Actively seek opportunities to engage with affected population
Identify and enlist the affected population to promote services
Serve as community liaison and representative with community organizations, stakeholders, providers, and town, city, and county governments regarding programming around home health, palliative, caregiver support, and homemaker services
EDUCATIONAL AND EXPERIENCE REQUIRED:
Bachelor's degree
5+ years successful program management experience including staffing and promotion
3+ years of experience in healthcare related field ideally in the area of home health or hospice care
Prior experience with volunteerism in the community
Must be proficient in MS Word & MS Excel and have aptitude for computers and working with various software
PREFERRED QUALIFICATIONS:
Knowledge and experience working in/with clinical, public health, &/or social service agencies in rural communities
Experience working with disadvantaged/underserved or aging populations
Experience with financial planning and strategic planning
